REC-001096
active2006–2013 NISSAN PATROL
Nissan has identified that due to lateral load on the accelerator pedal, the contact pressure of the sensor brush to a terminal on the circuit board of the subject sensor may deteriorate. As a result, the engine Malfunction Indicator Lamp (MIL) will illuminate, and the vehicle could go into the fail-safe mode, which would cause the accelerator operation control to limit vehicle acceleration. In extreme cases when the engine is idling or the vehicle is coming to a halt, air intake may become insufficient and the engine may stop running.

REC-003516
active0 LEXUS RX400H
Due to variations in the circuit characteristics of the Intelligent Power Module, higher operating temperatures may result in internal solder degradation. Eventually this will cause heat damage to the Intelligent Power Module with illumination of various warning lights on the instrument panel. In most cases, the vehicle will enter a fail-safe mode, resulting in reduced driving power in which the vehicle can still be operated for short distances. In limited instances, the fuse of the power supply circuit could operate, causing the hybrid system to shut down and resulting in the vehicle stopping whilst being driven.

In this case (assuming the parking brake has not been applied), the vehicle may roll (depending on parking condition e.g. if parked on an incline). For the sake of clarity, this issue does not affect proper operation of the hand brake and if the hand brake is applied the car will not roll.
